Internship Approval Agreement Process

Starting in spring 2017, all Internship Approval Agreements for THM3185 – Internship I and THM4185 – Internship II will be submitted electronically for review.  No paper approval agreements will be accepted.

Step 1:  Download and complete the appropriate paperwork. [Note: Please complete these files within Adobe Acrobat, not a web browser to ensure the paperwork saves correctly.]

Step 2:  Save your approval agreement as LastName – Spring 2017 – Course (i.e. THM3185 or THM4185).  Please merge your current resume as part of the PDF.  Not sure how to merge your resume into a PDF?  In Adobe Acrobat, select Create, then Combine Files into a Single PDF.  From there it will prompt you to select the files you want to merge.  Make sure the Approval Agreement is the first page of the document.  See screenshot below of where to find this option.

Step 3:  Upload your approval agreement below with your resume included.

Upon submission, please allow for 3-5 business days for review and approval.  You will receive an email when your paperwork has been reviewed.
